Reporting Support Worker concerns

In an attempt to catch the Support Workers who are serial shift droppers, late arrivers, early leavers, unresponsive and those who are unreliable with signing in and out of services correctly, we have developed a form for our scheduling team to easily report on these instances. If you become aware of a staff member doing any of the above, please use this form to report it. All other feedback or complaints should be emailed to HR as normal.
